Key Responsibilities

The Hospital Clerk will be responsible for a range of tasks, including:

  • Rendering birth registration services to clients.
  • Receiving notice of birth and relevant supporting documents.
  • Completing DHA 24 forms.
  • Performing online verification of informants or taking full sets of fingerprints when online verifications are unavailable.
  • Capturing applications on the National Population Register.
  • Submitting applications for quality assurance to a supervisor.
  • Confirming informant’s details and printing birth certificates.
  • Signing and issuing birth certificates as delegated.
  • Ensuring the informant signs the register to acknowledge receipt of the birth certificate.
